Discontinuation Notice

In the case that you wish to cease lessons at the Conservatorium of Music Ltd (Conservatorium), please complete the following Discontinuation Notice form. This will both advise our administration team of your cessation and greatly assist us to improve our services.

For ensembles, the full terms fees are due and payable if discontinuing after the commencement of a term. 

If withdrawing from individual lessons after the commencement of a term , 2 weeks notice must be given and payment for the subsequent 2 weeks is still required.
